    De l’aveu même des responsables des établissements de santé, le déclenchement et le déroulement des contrôles portant sur la tarification à l’activité (T2A) sont encore mal vécus par les équipes, en particulier au sein du département de l’information médicale (DIM). Relayée par les fédérations hospitalières, la colère des établissements se fait toujours entendre.

    Cette colère ne porte bien évidemment pas sur le principe des contrôles, ni même contre les organismes d’assurance maladie, mais sur la mise en oeuvre de ces contrôles et sur une application stricte des textes dont la compréhension n’apparaît pas d’une limpide clarté, exposant en cela les hôpitaux et cliniques à une insécurité juridique croissante.

      Je ne pense pas que les contrôles aient changé dans leur forme : les contrôleurs cherchent dans le dossier un élément justifiant le codage (la cotation), ils n’iront « jamais » contester la réalité de cette information. C’est dans l’incohérence entre le texte des éléments médicaux et le codage qu’est la faille.

      Comme souvent en terme juridique, la réalité n’est pas l’élément déterminant.

      Par exemple en chirurgie, tout acte codé doit figurer de façon explicite dans le compte rendu opératoire. Qui doit aussi faire mention du DP. D’éventuelles diagnostic associés (CMA qui vont changer le niveau du groupe GHM) doivent être trouvables aisément dans le dossier et même dans le CRO si elles sont connues initialement. Les complications ultérieures se retrouvent en général aisément au moins dans les transmissions des infirmières. (rétention urine: pose de sonde urinaire: passage en classe II)

      le problème habituel des établissements (surtout publics ex DG,mais aussi ex OQN) est une sous cotation des CMA, qui sont difficiles à repérer à posteriori.

Mais à mon sens, la préparation doit avoir lieu en amont, dès la création du dossier. Et l’effort d’éducation doit porter sur les praticiens (les infirmières sont TOUJOURS très disciplinées).

      L’informatisation du dossier médical est une aide précieuse à la vérification des codages, pour peu que le logiciel et son interface soit pertinente, et permettent des « vues » prévues pour: rapprochement sur la même page du CRO et des codes, voire d’une case des transmissions infirmières « événements inhabituels ».
      Une bonne informatique va faire gagner un facteur 100 dans le temps nécessaire à la vérifications des codes, ce qui en pratique fait passer de l’absence de vérification à une vérification réelle, et a des contrôles négatifs. C’est un point fondamental qui est trop peu souvent vérifié lors du choix d’un logiciel.
